Coordinated care for ongoing conditions

Our goal is to help every patient reach the best possible health. For people living with a chronic or complex condition, that means proactive, organised care rather than simply responding to problems as they arise. Our doctors and trained nursing staff develop care plans that are tailored to your individual needs and set out clear, practical steps.

GP Management Plans

A GP Management Plan helps you and your doctor map out how a long term condition will be managed. Where extra support is needed, your plan can include a Team Care Arrangement, which may make you eligible for subsidised visits to allied health providers such as physiotherapists, podiatrists, dietitians and exercise physiologists.

Our team regularly supports patients living with conditions including diabetes, asthma and other respiratory disease, heart disease, high blood pressure, osteoporosis, arthritis, kidney problems, stroke recovery and obesity. Regular reviews keep your plan up to date as your needs change over time.
