For people living with a chronic or complex condition, that means proactive, organised care rather than simply responding to problems as they arise. Our doctors and trained nursing staff develop care plans that are tailored to your individual needs and set out clear, practical steps.

Care plans made simple

A GP Chronic Condition Management Plan is a written plan you and your doctor prepare together. It sets out your health goals, the treatments and services you need, and how your care team will work together. With a plan in place, eligible patients can access Medicare rebates for visits to allied health providers such as physiotherapists, podiatrists, dietitians and exercise physiologists. From 1 July 2025 these plans replaced the older GP Management Plans and Team Care Arrangements, and if you have one of the older plans we can transition you across at your next review.
